If you are an admitted Add-on Endorsement student through CPE who would like to register for their capstone practicum, EDCN 5730, please complete this application. Applications submitted after 10/31/23 will be WAITLISTED, and offered a place in the course should one become available. 

When prompted, enter the following information: Course: EDCN 5730 Section: 322

1. Field Experience Explanation for Candidates: Extra information clarifying the expectations of the course. 

2. Mentor Information Sheet: Please distribute this to your potential mentor teacher to provide clarification on their role. It does not need to be returned. 

3. Mentor and Administrator Agreement forms: After these forms are completed by your mentor and administrator, please upload as part of your application. Dates and signatures must be current.
