Fall 2025 Field Experience (EDCN 5730)

If you are an admitted Add-on Endorsement student through CPE who would like to register for their capstone practicum, EDCN 5730, please complete this application. Applications are due 7/31/2025 and must be completed to be considered. If we have capacity to consider late submissions, we will post an update here after 7/31/2025, but we strongly suggest submitting on time! 

1. Field Experience Explanation for Candidates: Extra information clarifying the expectations of the course. 

2. Mentor Information Sheet: Please distribute this to your potential mentor teacher to provide clarification on their role. It does not need to be returned. 

3 Mentor and Administrator Agreements: After these forms are completed by your mentor and administrator, please upload as part of your application. 

• Please note it is part of your professional responsibility as an educator to prepare and submit a completed application. Our CPE staff does not have capacity to collect and file partial application materials via email on your behalf. 
• Note that priority registration will be given to students who have completed all other coursework in their program. Students MAY submit applications with coursework still pending, but these applications may be waitlisted. 
• All dates and signatures must be current, do not re-use forms from a past application. 

Registration and payment occur separately after this application has been completed and approved.
