Fall 2024 Field Experience (EDCN 5730)

If you are an admitted Add-on Endorsement student through CPE who would like to register for their capstone practicum, EDCN 5730, please complete this application. 

1. Field Experience Explanation for Candidates: Extra information clarifying the expectations of the course. 

2. Mentor Information Sheet: Please distribute this to your potential mentor teacher to provide clarification on their role. It does not need to be returned. 

3 Mentor and Administrator Agreements: After these forms are completed by your mentor and administrator, please upload as part of your application. 

• Please note it is part of your professional responsibility as an educator to prepare and submit a completed application. Our CPE staff does not have capacity to collect and file partial application materials via email on your behalf. 
• Note that priority registration will be given to students who have completed all other coursework in their program. 
• All dates and signatures must be current, do not re-use forms from a past application. 

Registration and payment occur separately after this application has been completed and approved.
